In some cases, it is important for your child to have reports that demonstrate the progress they have achieved or the plans you may have for their upcoming therapy and care.

NDIS Reports

NDIS reports have been created to ensure it provides all the required information that your NDIS planner will be looking for. Whether you are doing your child’s first plan, annual planning, plan review or plan appeal, we work with you to ensure that the report covers the history of your child, their progress, and your future goals for them.

Please carefully consider the life goals you are putting forward as part of your child’s plan, as they will be required in the report request. Our best results with NDIS reports is where you fully brief us on what you are hoping to achieve for your child so we can put as much detail as possible in the reports.

Please Note

Please note, these reports are complex and can take some time to prepare. Be aware of your child’s plan dates, and provide us as much notice as possible. Coordinated Kidz cannot guarantee we can meet a NDIS meeting date if we are not given adequate time to prepare a report.

You will find a copy of the cost for reports in the fees sections. In general, report fees can be paid for using your NDIS Funding however this may be dependent on your child’s individual plan and how your funding is currently allocated.

Intensive Summary Report

Every intensive that is completed at Coordinated Kidz comes with an Intensive Summary Report, which will be an additional charge on your Intensive invoice. 

This report will summarise the activities undertaken by your child during the intensive and the progress that was made against their goals. This report will automatically be sent to you within 3-6 weeks after the completion of your intensive.

Whilst the Intensive Summary Report contains useful information about your child’s progress during that specific intensive, we recommend completing a full NDIS Report for upcoming NDIS Reviews or planning meetings.

Payments & Cancellations

Payment of service is required at the time of a consultation, with the exception of those clients who might be under NDIS or another insurance agency. 

Payment methods include:

  • Private Payments 
  • Private Health Insurance
  • Medicare
  • NDIS
  • Third Party Insurance

NDIS Payments

Self Managed

If you are privately funding or self-managing NDIS funding for the intensive, you are required to place the deposit amount within seven (7) business days of your intensive invoice arriving. This amount will go towards the total of the Intensive. Receipts will be emailed to you following the completion of all payments. Please note, we are unable to amend dates, therapist names, on any invoices after the appointment has been completed. You are not able to use multiple Government or private health funding sources to pay for an individual therapy session if you are claiming via the NDIS. You will also be required to Sign Service Agreement, prior to commencing your sessions.

Plan Managed

If your child has a Plan Management company administering their NDIS plan, you must complete and return a Service Agreement within seven (7) business days of receiving your invoice or in advance of your service, supplying in writing your Plan Management company’s full details and direct contact. 

Your Plan Manager will need to email Coordinated Kidz to confirm that funding is available and secured to cover the intensive/planned sessions and a then deposit will NOT be required. Coordinated Kidz will work with your Plan Management company to confirm terms of payment at the end of the intensive or following your service. If we do not receive Plan Manager information or confirmation of funds within seven (7) ahead of your weekly sessions, or your child may be moved back on to the waitlist.

NDIS Managed

If your child has a current NDIS Agency Managed plan that will cover the therapy session dates of the invoice, you must complete and return a Service Agreement within seven (7) business days ahead of your weekly services. A service booking will then be made by Coordinated Fitness (Coordinated Kidz) on your behalf and a deposit will NOT be required. If for any reason Coordinated Kidz is unable to make a service booking, we will be in touch and you may have to find another way to secure your intensive.

Rebates

You may be eligible for rebates for your child’s session through Medicare with an eligible GP Management Plan or Team Care Arrangement. 

Additionally, you may also be able to claim a portion of your therapy sessions through your private health insurer. We have facilities at the clinic where you can swipe your card at the time of payment.
